چکیده

BACKGROUND Incomplete or delayed diagnostic resolution of breast abnormalities suspicious for cancer may contribute to poor breast cancer outcomes. We examine system predictors of timely diagnostic resolution in low-income, mostly Latina women with breast abnormalities at two Los Angeles County public hospitals. METHODS We collected medical record data on 1,671 women. The outcome--timely diagnostic resolution--was defined as receipt of definitive diagnosis (malignant or benign) within six months of the index referral. Predictors of the outcome were examined with bivariate and multivariate logistic regression models. RESULTS Only 56% of women received timely diagnostic resolution; however, the proportion was higher at Hospital B (67%) compared to Hospital A (51%). The effect of predictors differed by hospital. At Hospital B, the odds of timely diagnostic resolution were lower for women with self-identified abnormalities (P <.033) and higher for women referred to Radiology rather than Surgery for their first appointment (P <.005). At Hospital A, the odds of timely diagnostic resolution were higher for women with self-identified abnormalities (P <.043) and lower for women referred to Radiology rather than Surgery for their first appointment (P <.001). CONCLUSIONS Rates and predictors of timely diagnostic resolution differed by hospital. System barriers may be a function of referral processes and patient tracking. Interventions may need to be tailored to a hospital's unique culture.

Low-income women with abnormal breast findings: results of a randomized trial to increase rates of diagnostic resolution.

BACKGROUND Timely diagnostic resolution of abnormal breast findings represents a critical step in efforts to reduce breast cancer morbidity and mortality. Yet, follow-up rates among resource poor populations are not optimal. Efforts to mitigate this disparity are needed. We report results of a randomized trial assessing the effectiveness of a patient support and navigation intervention in incre...

[Timely care after an abnormal mammogram among low-income women in a public breast cancer screening program].

BACKGROUND Since 1990, the National Breast and Cervical Cancer Early Detection Program (BCCEDP) has funded breast cancer screening and diagnostic services for low-income, underinsured women. Case management was implemented in 2001 to address barriers to follow-up after an abnormal mammogram, and free treatment was introduced in 2004. However, the effect of these policies on timeliness of care h...

Family physicians expedite diagnosis of breast disease in urban minority women.

BACKGROUND The specialty of a patient's usual provider of care is associated with use of mammography and stage of breast cancer diagnosis. It is unknown if specialty of usual provider of care affects time from discovery of a breast screening abnormality to diagnostic resolution. METHODS Retrospective chart review study of 546 women who had breast biopsies at an urban public university hospita...

Lasting Effects of the Breast and Cervical Cancer Early Detection Program on Breast Cancer Detection and Outcomes, Ohio, 2000–2009

INTRODUCTION The National Breast and Cervical Cancer Early Detection Program (BCCP) in Ohio provides screening and treatment services for uninsured low-income women aged 40 to 64. Because participation in the BCCP might engender greater self-efficacy for cancer screening, we hypothesized that breast cancer and survival outcomes would be better in BCCP participants who become age-eligible to tra...

Breast-cancer early detection in low-income and middle-income countries: do what you can versus one size fits all.

In general, rates of breast cancer are lower in low-income and middle-income countries (LMCs) than they are in more industrialised countries of North America and Europe. This lower incidence means that screening programmes aimed at early detection in asymptomatic women would have a lower yield--ie, substantially more women would need to be examined to find a true case of breast cancer. Because ...
